Rehabilitation

Rehabilitation services at Homeland Center provide physician-directed care to help residents recover from conditions that require short-term rehabilitation. Rehabilitation services are available to all Homeland residents in Personal Care, Skilled Care and Memory Care settings, as approved by their insurance.

Our clinical team works closely with rehabilitation specialists and residents to identify goals and follow an individualized treatment plan.

Rehabilitation may include physical therapy, occupational therapy and/or speech therapy. Examples of commonly eligible conditions for rehabilitation include mobility training, fall prevention, arthritis, head trauma, hearing loss, vision loss and swallowing disorders.

Homeland Center residents may receive rehabilitation services in their rooms or in a specialized therapy center at Homeland.
