HomeHealth is a physician-ordered plan of care provided at home.

This plan is monitored and implemented by a Homeland team of medical professionals including nurses, physical/occupational/speech therapists, social workers and Certified Nursing Assistants (CNAs).

Staff has expertise in wound care/healing, cardiac care, palliative care, fall prevention post orthopedic care and rehabilitation services.

This type of service is often funded by the individual’s insurance coverage or Medicare.
