Services
SERVICE AREA: Dauphin, Cumberland and York Counties. Nursing: Each hospice patient has a Registered Nurse Case Manager assigned to them. The RN Case Manager coordinates the overall care of the patient, and collaborates with the physician and all members of the hospice interdisciplinary team. The skilled nursing staff includes Registered Nurses and Licensed Practical Nurses. They work as a team with the Registered Nurse Case Manager to ensure the comfort and symptom management of the hospice patient. A Registered Nurse is available 24 hours a day, 7 days a week. Home Health Aide: Home Health Aides are available to assist with personal care. They are also available to assist with light housekeeping related to the patient’s care. Hospice Medical Director: The Hospice Medical Director is available to work with your attending physician as a resource for optimum pain and symptom management to meet the goals of the plan of care. The Hospice Medical Director along with the attending physician will certify and provide ongoing review regarding the patient’s eligibility for hospice care. When necessary the Hospice Medical Director is available to intervene to ensure the patients receives the appropriate treatment that supports the patients care plan and falls within hospice care guidelines. Attending Physician: The patient must remain under a physician’s care. Your physician will be a part of the hospice team and will participate in establishing the plan of care. Your physician will continue to provide medical orders and work with the hospice team to direct your care. Social Work: The patient’s terminal illness may result in many challenges and losses. The social worker is available to provide emotional support, assistance with availability of resources/ services from the community, and grief counseling. Spiritual: Spirituality may be a very important part of the patient and/or family’s life. Spiritual support is available to any patient or family member who requests this support. Homeland Hospice is aware that every individual’s belief is as different as every person. The Spiritual counselor will not force his or her own beliefs on the patient or family members. Instead this person will meet you where you are and use your own belief system to support you with this difficult time. Medical Equipment/Supplies: Equipment and supplies such as hospital bed, bedside commode, oxygen, wheelchair, etc are available as related to the terminal illness. Most equipment is covered by Hospice but may depend on the patient’s insurance coverage. Pharmacy: Medications are provided to the patient under the terminal diagnosis. The medications are delivered to the patient’s home or from a local pharmacy in certain areas. Support Team Members: The patient may require other assistance from other therapies. These are available from a physical therapist, occupational therapist, speech therapist, and dietician. Bereavement: Bereavement services are provided for 13 months to the loved ones/ caregivers after the death of the patient. These services may include phone calls, mailings, support groups, or visits if needed.
